Use a limited type:

type Dirty_Info is limited record
    Value : Boolean := False;
end record;

procedure Refresh (Object : in out T; Dirty : in out Dirty_Info);
...
    if Dirty.Value then
...
    Dirty.Value := True;

Parameters of limited types are passed by reference.
