Is there an agreed upon name for the three forms used
in the subject line, viz. if_expression, case_expression,
and quantified_expression?
Or is "conditional [expressions] and quantified
expressions" the closest phrase, e.g. to be used in
a headline?

Yes.
Conditional expressions: (if ...) and (case ...)
Quantified expressions: (for all ...) and (for some ...)
