Where can I find IEE695 file format

Does anyone know where can I get a copy of the IEE695 file format
specification for microcomputer object file format. I tried to purchase
it from the IEEE, but they, amazingly, have lost a key page from their
copy!!

When I last picked up a copy of a document which describes IEEE695 with the
HP and MRI extensions, I got it from

  ftp://hpcsos.col.hp.com/dist/ieee-695/695spec4r1.ps   and
  ftp://hpcsos.col.hp.com/dist/ieee-695/695apps4r1.ps

These files are dated May 12 1994 and have sizes of 173235 and 695631
respectively.
