Did you open (as out_file) the file you want to associate to Set_Error?
Here an example that's working:

Sorry, it seems like stderr redirection is not common between Ada.Text_IO and System.IO. I wanted to redirect output from GNAT.Task_Stack_Usage. What I'd really like is a passive iterator over the task entries, so that I can smoothly send output from our embedded to our Trace Application running on a PC.
