I recall reading something recently about a one line code patch that was not
tested, but instead put into immediate service (the phone company?) and
caused a disaster. The patch just set the value of an identifier, but the name
was mistyped (something like Rl instead of R1). I cannot remember the source,
but would like to cite it.
Can anyone tell me the particulars of this case, and supply a published
reference?
