  Ada - Quality and Style
  Guidelines for Professional Programmers
developed by the Software Productivity Consortium
  published by Van Nordstrom Reinhold

The book is focused mostly on Ada programming style, but
includes a 32 page chapter on portability issues.
The format is a set of guidelines, followed by rationale, examples, notes
and exceptions. The text explicitely avoids lexical issues

Many of the portability issues deal with the floating point system,
  due to the authors interest.

Though certainly not perfect, I highly recommend it.
