Actually I have come across a couple of editors/reviewers requiring that style (all languages all-caps)---but I don't think this is the cause of the current mispellings. (It was some time ago now, perhaps for single font, no italics, no bold, typewritten documents, perhaps University theses, applications for grants, can't remember exactly---but I remember the requirement well, you can imagine why...)
