I don't recall doing anything special to get the C include files in the standard place, i.e. /usr/include, but this answer http://superuser.com/a/1014872 looks as though it's what I must have done. I've taken to installing just the command line tools, as Ondřej suggests, because I don't have any reason to use Xcode.
