Ada was standardized in 1983, and it is definitely NOT the case that no one thought about a revision till 1993. On the contrary, what happened is that people *did* start thinking about a revision in the time period consistent with the five year rule -- this rule does not say you need a standard every five years, just that you should start to think about it. But the process from starting to think to standardizing Ada 95 was a very long process (6-7 years, I don't have the exact starting point).
