It's not always the programmer's fault; you're being a little unfair here. Many times it's the programmer's peers or manager's or lead's fault. There are many reasons for not following established standards. To put the blame on a programmer and fire them is unqualified. You need to find out where the problem really is. Now, if there is a process, and there are deliverables expected, and some programmer consistently refuses to deliver, then yes, they may require disciplinary action. > The industry simply cannot afford such a level > of incompetence.
