There's an interesting paper on error rates at http://panko.cba.hawaii.edu/ssr/Mypapers/whatknow.htm It focuses on spreadsheets, but mentions data from other fields. Humans seem to run around 1-5% errors, with the low side being uncorrected mechanical errors (typos, letting the car drift outside its lane, etc) and the higher side for logical or judgement errors. Humans are also highly overconfident in their estimates of what a good job they've done.
