Arianne 5 failed because of bad engineering decisions. The choice of Arianne 4 components, proven and tested, for Arianne 5 would have been fine if they were both the same design. They weren't. Using the wrong component, however good that component might be, is equivalent to a physician prescribing a perfectly good medication to someone with a medical history for which it is contraindicated.
