Robert Firth gave a clear, short (98 lines) discussion of this topic that 
was the best of the exchanges last June about called subprograms as 
parameters.  If anyone wants a copy, send me a message and I'll send it.  
(If there's more than, say, a half-dozen requests I'll post to the net.)

Be warned that I sometimes have message bounced from some of the net
