For mixed case you can also preserve existing capitalisation, so that e.g. text_IO would be changed to Text_IO rather than Text_Io. Existing line breaks are preserved, and where an "unexpected" line break occurs the indentation of the following line is left unchanged, so that reformatting again will not introduce any further changes.
