The process which leads to a standard always has to considder the evolution process for standards by establishing procedures for refinement and reworking. I guess ISO/ITU/ETSI just name it do have such procedures. The problem is a different one, If you invested a lot effort in implementing a standard, then you are not automatically interested in revising it unless is simply does not work.
