If the reviewer needs to resort to saying "capital-v variable" for example, then any advantage of case insensitivity can be quickly lost. As an anecdote, I once worked on a code generator for an OOA tool which had its own specification language. Two features were: case sensitivity, and variables declared by first usage. You can guess the number of times duplicate variables got created.
